Does the Toronto FUE hair transplant leave a scar?

It’s important to communicate clearly to prospective patients that there is no hair transplant surgery that is entirely scar-free. What is increasingly possible and desirable, however, is a procedure that results in nearly invisible scars. The FUE hair transplant approach leaves such tiny micro-graft extraction marks that they aren’t readily seen by the naked eye. A skilled transplant specialist will remove donor grafts from the back of the head in a randomized way with punches measuring less than 1 mm in diameter. The remaining hair is dense enough that the little white dots that result aren’t seen even if the hair is cut quite short.

What is FUT?

Follicular Unit Transplantation

Also referred to as the “strip” hair transplant technique, FUT requires a strip of the scalp to be surgically extracted and dissected under microscope. This traditional process allows for high yields of follicle grafts but leaves a linear scar at the back of the head. The healing time needed to ensure the long incision is closed and protected from infection will be a minimum of 2 weeks.

The discomfort and related restrictions can be inconvenient as well. People who have visible FUT scars have the choice of wearing their hair long enough to provide cover, undergoing additional FUE transplants to fill the scar, or receiving scalp micropigmentation (SMP) coverage. Sometimes additional procedures are needed to fill in the crown with ideal density, and this results in further strip scarring.

By contrast, with FUE (follicular unit extraction) hair transplants, the donor area is clipped short or shaved, and then individual follicular grafts are extracted one by one. The incisions for this method are not long and do not result in the extraction of a strip of scalp. Instead, a tiny rotary punch device extracts just the small bunches of 1 to 4 hairs and a little surrounding tissue.

The holes are typically 0.8 mm to 1 mm in diameter, which means that no stitches or staples are needed to close them. Not even a bandage will be required, as the micro-incision openings close within a few days and heal quickly. As you can imagine, this minimally invasive technique has dramatically sped up the FUE recovery process.

This modern method produces a tiny hair plug that is then implanted into a hair-thin slit in the recipient area. When carried out by a skilled expert, the only scarring that remains is a randomized series of tiny dots, and they aren’t visible in most cases.

The technology used to transplant hair root grafts is only about 60 years old. Compared with facial and body cosmetic surgeries, that’s extremely recent.

Since the first documented procedure in 1952, methods have evolved into the realistic and natural-looking process that is sought after today.

The earlier grafts were roughly the diameter of a pencil. Yikes! Those giant hair plugs could be taken from areas of dense growth that were resistant to the DHT hormone that causes hair loss and then surgically transplanted to balding regions. The results were mixed, both in terms of medical take rate and the aesthetic effects created. This particular “hair plug” technique was the mainstay of hair transplantation all the way up to the 1980s, which is why some people today still cringe to remember rows of hair bunches planted awkwardly on balding heads.

Smaller mini-grafts became more prevalent in the mid-80s and utilized hand-dissected, smaller pieces taken from an excised strip. The smaller grafts containing only a few hairs did wonders for increasing the aesthetic success of the procedure, but the trade-off was a long, visible scar at the back. Because removing scalp tissue results in a tighter scalp, sometimes this tension on the healing surgical incision also leads to a widening and challenging scar.

Multiple attempts to improve on this technique have promptly failed on all fronts. They include laser implant site creation and the surgical transplantation of large sections of scalp. The strip method remained the procedure with the most pros and fewest cons until recent years. The old follicular unit extraction technique was perfected to allow the tiniest micro-follicle extractions and to implant them in micro-slits. With punches being quite superficial and as small as 0.8mm wide, the resulting trauma is greatly minimized, and the procedure can be done so skillfully that no sign of injury results.

Style matters

The implantation of grafts following the natural direction, density, and pattern of real hair is also incredibly important. This is one reason why micro-extractions should be carried out by a certified hair transplant surgeon, not an unlicensed technician. An artistic eye is essential to avoid creating scars and visible donor depletion. A realistic-looking hairline depends on it.
